Dr. Muhammad Salman Rais

Dr. Muhammad Salman Rais completed his Family Medicine training in Saint Louis, Missouri. He also has Master in Health Administration from Central Michigan University. Dr. Rais has extensive experience with assisted living, palliative care, Nursing Homes and long term care. Dr. Rais is looking forward to provide compassionate, comprehensive and complete care to patients with his staff.

Erin Warriner

My name is Erin Warriner and I am the new nurse practitioner that will be rounding with Dr Rais. I am excited to start a new journey in my nursing career. I was a nurse for 8 years on a cardiac unit at Mclaren Greater Lansing and I have been working as a nurse practitioner for the last 6 years for internal medicine. I love helping people and practicing medicine and look forward to helping people manage their chronic diseases. I am married and have 3 children all girls who keep me busy. I live in Dimondale. I also enjoy working out spending time with my family.

Sarah Young

My name is Sarah Young and I am a board-certified Adult- Gerontology Nurse Practitioner. In 2024 I obtained my Master’s of Science (MSN) degree from Spring Arbor University. However,I have been a registered nurse since 2006. At that time, I obtained my Bachelor’s of Science (BSN) from Lake Superior State University. My nursing career includes acute, subacute, and long-term care. During my nursing career, In 2019 I became a board-certified wound care nurse. I obtained my training through Cleveland Clinic’s R.B. Turnbull Jr. WOC Nursing Education Program.With my experience and passion for the geriatric population, I am honored to be one of the providers on your care team.In my free time, I have several hobbies. I enjoy weight lifting, running, boating, and spending time with my family, I have been married since 2016 to my husband Ryan. Together we have two young boys, Greyson and Noah.

Jessica Troyer

Jessica Troyer, DNP, APRN, AGCNS-BC, is a graduate of the doctoral nursing program at Michigan State University. Jess has a passion for gerontology, with several years of experience in long-term care. Her philosophy is to grant those under her care the most quality and independence possible, and to include family, culture, and spiritual considerations in patient care plans. Jess lives close to her parents and their family farm in Freeport with her kids and spouse.
